TELE-EDUCATIONAL SERVICES (FGL Class Check-in/Sessions or Coaching) INFORMED CONSENT CHECKLIST

for the Four Gifts of Love^® Online Institute

Prior to starting tele-education (video-conferencing) services, we (husband/wife; "subscriber") discussed and understand the following: 

• There are potential benefits and risks of video-conferencing (e.g. limits to subscribers' confidentiality) that differ from an in-person meeting.
• The importance of your privacy still applies to tele-education services, and although recording the sessions is never allowed, within the group sessions, it cannot be guaranteed that all subscribers will follow the strict guidelines.
• It is important to be in a quiet, private space that is free of distractions for the video-conferencing services.
• If the coach determines a subscriber is being abusive or causing a distraction, the coach will issue one verbal warning. If the behavior continues, the subscriber's video and audio will be disabled. A follow-up discussion will be required to determine if video-conferencing services can continue. 
• It is important to be on time.
• If there is a need to cancel or change the check-in/session appointment, the coach must be notified via email at least 24 hours before the scheduled meeting. If the coach is notified in less than 24 hours, the check-in/session will be forfeited. 

Technology Needed:

• We agree to use the video-conferencing platform (ZOOM, Microsoft Teams, or DOXY.ME) for the class check-ins/sessions or coaching sessions.
• We will download the ZOOM, Microsoft Teams, or DOXY.ME application onto the device used for the meeting and secure the needed internet connectivity in preparation for entering the ID and password given by the coach.
  
• It is recommended that subscribers delete their last name under the video before entering the meeting if it appears.
• It is preferable to use a webcam or smartphone during the check-in/session.
• It is important to use a secure internet connection rather than public/free Wi-Fi.
• In the event of any technical problems, a backup plan should be discussed.
  

Correspondence with your coach:

• The coach will be using the email address provided from the FGOI site registration for Class correspondence.
• The coach does not use a secure email or phone program for correspondence. Therefore, the subscriber should be aware that any electronic exchange is not guaranteed to be confidential and could be intercepted or sent to the wrong address. In addition, response time may take up to two business days.
• The check-in/sessions and email messages will include discussion about the concepts and upcoming assignments. No private/intimate information should be shared during these times.

Participation:

• The tele-education services provided are educational only and not intended as therapy or to replace therapy.
• The experience in the Four Gifts of Love^® Class or coaching may create new perceptions and a range of feelings that at times may include uncomfortable emotions. It is always an option for each individual to choose to participate or not to participate based on one’s own comfort level.

No Surprises Policy:

• Costs are transparently presented before purchase.
• No refunds.
  
• Services must be used within the number of months that the respective learning format identifies or the remaining check-ins/sessions will be forfeited, unless an extension is purchased.
• Subscribers are allowed a maximum of two rescheduled check-in/sessions. A request to reschedule must be made at least 24 hours before a scheduled check-in/session. The make-up session must be scheduled within two days, subject to the coach's availability. Otherwise, the session will be forfeited. 
  
• PayPal will send an email of payment confirmation which will be the ONLY invoicing/receipt offered.
